I certify that the information
given by me is true and complete and agree that Firebird Education assists
in finding me a post only on the understanding that my acceptance of any
new post will not violate contractual agreement with another employer.
I understand that the posts for which I may apply through Firebird Education
are exempt from the provisions of section 4(2) of the Rehabilitation of
Offenders Act 1974. I confirm that I have fully declared to Firebird Education
any convictions, cautions or bind-overs, including any that would be regarded
as spent under the Act in other circumstances. As I am or have been, resident
in the UK I understand that if selected for appointment, I will be subject
to an Enhanced Disclosure by the Criminal Records Bureau. Due to the nature
of the posts I am applying for through Firebird Education it will also
show relevant information from the Dept of Health and DfES lists of people
barred from working with children.
